How To Order TB Research Materials

Step 1: Register with the TB Contract

  • Download the Registration Form here.
  • Enter your information using Adobe.
  • Print the form and fax to 970-491-1815, attn: Ms. Megan Lucas. You may also e-mail the completed form to megan.lucas@colostate.edu. You are encouraged to keep a completed copy for your files by either printing the form or saving it to your hard drive.
  • After your registration form has been received and your information is entered in our system you will receive an email with your registration number.

Step 2: Login and fill out an order

  • Once you have a valid registration number, you can create a login and place an order.
  • Create a login by going to Login & Ordering. Please use the email address that you provided on your registration form.  Be sure to share your login information with any members of your lab who will be ordering materials.
  • Place your order for materials, being sure to check your user information to make sure that the address and shipping account are correct.
  • You will need to provide a Federal Express or DHL account number in your user information that can be used to ship the materials, as this contract does not cover the cost of shipping and cannot ship items collect. Federal Express accounts are preferred, as DHL does not accept dry ice shipment. If you do not have a Federal Express or DHL account, you can open an account online at www.fedex.com or www.dhl.com. UPS accounts are also acceptable. Other means of covering shipping costs are available; these are limited to international recipients from select countries where these accounts are not available.
  • If ordering more than the default quantity of a material, email a detailed justification, including experimental design, amount of material used in each assay, and preliminary results where applicable, to Dr. Karen Dobos-Elder. Be sure to include the name of your principal investigator. Unless a research justification is provided, you will automatically receive the default quantity.
  • All orders are reviewed on Thursday morning of each week. You may check the status of your order from the Login & Ordering page.
  • Please note: In order to accommodate the volume of orders received by the contract, we must limit the number of packages received by the laboratory of each principal investigator to five shipments per year. For this reason, there is also a limit on frequency and you should allow 4-6 weeks between shipments.

Step 3: Submit a Materials Transfer Agreement (MTA)

  • Individuals associated with academic, government, or private research institutions, or a non-profit company, download the Uniform Biological Materials Transfer Agreement. If you work for a for profit company, please contact Megan Lucas to obtain the appropriate MTA.
  • Fill out the form using Adobe and print three copies of the MTA (one copy each for: our records, CSU's Office of Sponsored Programs, and your institution)
  • All three forms must be signed by the laboratory's pricipal investigator and an official representing the requestor's institution (VP, Director, Dean, Dept. Head, or lawyer representing the institution, etc.).
  • All three signed originals must be mailed to:

  • Faxes and photocopies will not be accepted. We must have all three original signatures.

Step 4 (if applicable): Complete a BSL-3 Certification Letter, Genomic Array Application, Justification, or JHU Materials Transfer Agreement

  • If ordering live Mycobacterium tuberculosis cells: download, complete and return a BSL-3 Certification Letter that has been signed by your institution's biosafety officer.
  • If ordering microarray slides: read the guidelines regarding genomic arrays, then complete and submit a Genomic Array Application.
  • If ordering more than the default quantity of a material: you must submit a detailed justification, including experimental design, amount of material used in each assay, and preliminary results where applicable, to Dr. Karen Dobos-Elder.
  • If requesting transposon mutants: submit a BSL-3 Certification Letter that has been signed by your institution's biosafety officer, and a Johns Hopkins University Materials Transfer Agreement. We must receive four original signed MTAs (one each for: our records, CSU's Office of Sponsored Programs, JHU, and your institution)
