Access to the Colorado Supplemental Application and the DVM Admissions Portal
The College of Veterinary Medicine and Biomedical Sciences (CVMBS) at Colorado State University requires electronic submission of both the VMCAS Application (Veterinary Medical College Application Service) and the Colorado Supplemental Application (CSA).
2012 Colorado Supplemental Application (CSA) - The CSA is now closed and the 2013 CSA will open mid July 2012.

2012 VMCAS Application - The 2012 VMCAS Application is now closed and will open June 7, 2012.
Sample Colorado Supplemental Application - If you would like to view a copy of a sample application, we have posted a copy of last year's 2012 application. This may help as a reference for the 2013 application and it may also help for the purposes of reading, planning, and printing. Please know some of the fields will change and new fields may be added to the 2013 application. You can not fill out this version, nor can you submit this version. When evaluating DVM application(s), the Admissions Committee reviews a profile summary of your Colorado Supplemental Application and your VMCAS Application. We have posted a sample profile to show you the sequence of how your information is presented and read by the Admissions Committee.
