Come Spend a Night in the ER
History: "Rusty", canine, neutered male, 11-years old. Weakness, vomiting, collapse, receiving Carprofen for arthritic coxofemoral joints.
Physical Examination:
Temperature = 102.3oF (39.1 C), pulse = rapid, weak, respirations = 100
breaths/minute, weight = 80 pounds (36.7 Kg). Mucous membranes are pale and
tacky; skin turgor appears to be decreased; assessment of dehydration is 7%.
Working Diagnosis:
Dehydration, shock, possible renal insufficiency, evidence of diarrhea, liver
failure secondary to Carprofen toxicity (?).
Emergency Treatment: Intravenous fluids (Normosol-R) at one-quarter shock volume IV.
Diagnostics:
Emergency Treatment: Intravenous fluids, lasix, dopamine.
Follow-Up: Due to the grave prognosis and lack of urine production, "Rusty" was euthanatized.
Necropsy Findings: Kidneys had small infarctions, very large aortic thrombus, hemangiosarcoma in the spleen with metastasis to the lungs.
