To order your uniform:

1.  Call Ellen, Army-Navy Store in Atlanta, at 770-426-7702 and ask for 2 long sleeve shirts and 2 pair of pants, plus black military style boots.  She will know what the VMAT teams have ordered.  She also has long underwear, socks, etc.

2. Call Dreama, at T-shirt Attic in VA, at 540-667-2351 and order 2 polo shirts with Dr. John Doe embroidered over the right pocket, actually there is no pocket, and the NDMS and USPHS emblems over the left pocket.  I will send you a picture of the location of the patches.  Also, order 2 VMAT T-shirts in your size.

3. Call Galls 800-477-7766, order 3 name strips (last name only), #D-UA025, they are navy with white lettering.

4. Your Team Leader or Admin. Officer will supply you with the NDMS patch, USPHS strip, VMAT patch, and VMAT rockers to attach to your uniform.  (A web page will be developed to illustrate where the patches are located on the uniform.)

Please purchase these items as soon as possible and prepare all your personal supplies for deployment.  Also, recommended is to pack a 3-day supply of food and water.

Water bottles with a filter may be purchased from CLEARBROOK  at 334-660-2581 (http://www.911water.com/bs_01_cb.html)  These filter bottles will remove everything except viruses.
