SHIPPING INFORMATION 

With each shipment, please include the following:

	Clinical Lab Form, Multiple Sample Lab Form, or Mare Pregnancy Test Lab Form
	At least 1 ml serum for each hormone to be analyzed.  (It may be possible to utilize smaller volumes, but prior lab approval from the Lab is required
	Samples are to be sent in glass or polypropylene 12x75 mm tubes that are tightly sealed
	Ice Pack

Shipping Instructions:

Samples are to be shipped via overnight carrier (send chilled or frozen).  If shipped other than overnight, samples must be frozen or on dry ice.
